Determine Eligibility Status
You obtain a “Certificate of Terminal Disease” from your doctor
Once we’ve mutually decided that hospice care sounds like the right next step, you will need to reach out to your Primary Care Physician (PCP) or Specialist to obtain a written hospice recommendation or prescription.
Obtain Insurance Approval
We help you obtain Medicare and/or Medicaid Approvals
Once your doctor recommends hospice care, we’ll work with Medicare and Medicaid to help you confirm approval.
Clinical Care Assessment
We develop a personalized Care Plan for each of our patients
